I lost 3 people on my first and second playthrough. I even lost legion

First playthrough I lost one, but found out what I was doing wrong and changed the second team's leader and they survived.
Second playthrough I think everybody lived.
Insanity playthrough somehow everybody survived, even Zaaed, despite the fact I failed his loyalty mission.

If you Paragon it Vigil, you can still gain his loyalty.
